ATHLETIC FORMS & NOTICES
The following forms are distributed in the schools before each athletic season. You may also download the forms here and submit them to the athletic director or team coach. These athletic forms must be completed and returned to the Athletic Department before any student can participate on a Governor Mifflin sports team:
Physical examinations are required for any athlete wishing to participate in a sport. Physicals are offered by the Governor Mifflin School District prior to the start of each sports season. If student athletes opt to have a physical provided by the District during one of the four designated athletic physical dates, the physical is provided free of charge. Please check the District calendar as physical dates are announced throughout the year. Athletes my also opt to have a physical performed by a private physician. The forms below must be completed and returned to the Athletic Department before any student can participate on a Governor Mifflin Sports team:

Governor Mifflin School District is currently implementing ImPACT, an innovative program for our student-athletes. This program will assist our team physicians and athletic trainers in evaluating, treating and managing concussions.
PARENT/COACH COMMUNICATION PLAN
Both parenting and coaching are extremely difficult vocations. By establishing an understanding of each position, we are better able to accept the actions of the other and provide greater benefits to children.
All students who participate on our athletic teams will be assessed an activity fee. Students will be charged $ 75.00 per student for participating. For families who have multiple students who participate in activities, the total activity fee per family will be $ 225.00/per year. Please see below for the fee policy form and procedures for the 2016-2017 school year:
WAIVER REQUEST FORMS
In cases of financial hardship, parents of students in Grades 7-12 may request a waiver for the Student Activity Fee. Consideration will be given on a case-by-case basis.
