Athletic Forms & Publications
The following forms are distributed in the schools before each athletic season. You may also download the forms here and submit them to the athletic director or team coach. These athletic forms must be completed and returned to the Athletic department before any student can participate on a Mifflin sports team:

Tax Rebate Form
To ensure the safety of our students and to follow proper procedures and regulations necessary to make our volunteer program effective, all volunteers are required to obtain state clearances. The following guidelines include links to Criminal Background Check forms and Child Abuse Clearances.
Waiver Request Forms
In cases of financial hardship, parents of students in Grades 7-12 may request a waiver for the Student Activity Fee or Course Material Fee. Consideration will be given on a case-by-case basis. Please download the following forms and submit them to your child's school principal.
