Athletic Forms and Publications

Forms

The following athletic forms must be completed and returned to the Athletic department before any student can participate on a Mifflin sports team:

• Emergency Information Card *
• PIAA Comprehensive Pre-Participation Physical Form*
• PIAA Pre-Participation Recertification Physical Form* 
• Aquatic Registration Card

These forms are distributed in the schools before each athletic season.  Or, you can download the forms here and submit them to the athletic director or team coach.

The following health forms may be completed and returned to the Nurse or Food Services department if necessary:

• Request for Dairy Products and Food Substitutions*
• Request to Administer Medication During School Hours*

The following membership forms may be completed and returned to the appropriate school building or chairperson:

• Governor Mifflin Music Association Membership Form
