2013-2014 SCHOOL YEAR
Students who are 5 years old before September 1, 2013 are eligible to register for Kindergarten for the 2013-2014 school year only. District policy states that kindergarteners will be admitted to school only during the first two weeks of the school term, except for a child who is already registered in another public or state-approved private kindergarten program that may transfer at any time during the school year.

This process is for students who are new to the Governor Mifflin School District and need to be enrolled.

Registrations are by appointment only. Once you have completed the online registration, please contact the registrar, Mrs. Lisa Negri (610-775-1461, ext. 1105) to schedule an appointment.
In addition to the forms contained in the online registration process, the following documents are also required for your appointment:
PROOF OF RESIDENCY - (must provide 2 original proofs of residency)
- If a Homeowner:
A. Property Tax Bill, Mortgage Statement, or Recorded Deed in your name
And one of the following
B. County Registrar of Voters; or
Current vehicle registration showing address; or
Utility bill in your name of the current month showing address (no cell phone bills); or
Check stub/statement from wages showing address
- If NOT a Homeowner:
A. Signed lease or agreement (identifying the adult(s) and child(ren) as occupants)
B. Residency Affidavit (if residing with a Governor Mifflin resident)

PROOF OF IDENTITY
- Original or copy of birth certificate If parent names are not on birth certificate, a physician's letter stating parent's names/child's name and date of birth or United States Passport or Income Tax form that you claimed the child will be accepted
- Complete Immunization History or Baby Book
PROOF OF CUSTODY
- Court Ordered Custody Agreement (signed by Judge)
- Notarized letter from other custodial or biological parent (must be notarized and state other custodial/biological parent is aware child living with other parent and approve of the child attending Governor Mifflin School District)
- Last report card and/or unofficial transcripts (Unofficial transcripts for Middle and High school students)
SPECIAL EDUCATION (if applicable)
- Copy of current IEP or ER
- Request of Records Form
FOSTER/GROUP PLACEMENT (if applicable)
- Entrance Certificate
- Placement Letter from Agency
