Student Registration 2013-2014:
Describes the process for enrolling your child in the Governor Mifflin School District

Student Registration 2012-2013
Parents and guardians who wish to enroll their child in the Governor Mifflin School District should contact the registrar, Mrs. Lisa Negri (610-775-1461, ext. 1105) to schedule an appointment. The required enrollment packets are available at the Education Center, located at 10 S. Waverly Street, Shillington, PA 19607.
In addition to the forms contained in the enrollment packet, the following documents are also required for your appointment:
1. Proof of Residency
If you own your property:

Agreement of Sale (recently purchased a home in our district); deed; property tax bill or mortgage statement
OR

Utility bill and driver's license - must be current
If you lease your property:
-
Lease Agreement - must be current and signed by landlord and tenant
If you live with family or friends:
-
Residency Affidavit - must be notarized
2. Proof of Identity

Original or copy of birth certificate - If parent names are not on birth certificate, a physician's letter stating parent's names/child's name and date of birth or United States Passport or Income Tax form that you claimed the child will be accepted.
3. Health Information

Complete Immunization History

4. Proof of Custody (if applicable)

Child Custody Form - included in packet

Court Ordered Custody Agreement - signed by judge
OR

Notarized Letter from non-custodial - must be signed and notarized, stating non-custodial parent is aware child is living with other parent in our district and approves of the child attending Governor Mifflin School District

KINDERGARTEN REGISTRATION
2012-13 SCHOOL YEAR
Students who are 5 years old as of September 1, 2012, are eligible to register for Kindergarten for the 2012-13 school year. After September 1, 2012, students are eligible for Kindergarten if previously enrolled in a public or state approved private kindergarten program for the 2012-13 school year. Call or email Mrs. Lisa Negri (610-775-1461, ext. 1105) to schedule a registration appointment.
