May 30: Fall 2017 Athletic Physicals

To the parents/guardians of athletes participating in a FALL 2017 SCHOOL SPORT:

IMPORTANT! PLEASE READ ALL INFORMATION!

Governor Mifflin will be holding physicals for athletes (Grades 7-12) who will be participating in a FALL 2017 scholastic sport on Tuesday, May 30, 2017.  Physicals will be held in the high school main gym.  Times for each team to arrive are listed below.  Please try to adhere to these times!! 

4:00 p.m. Sr. High Football
4:20 p.m. Sr. High Field Hockey & Sr. High Girls Tennis
4:40 p.m. Sr. High Girls Soccer, Sr. High Girls XC, Sr. High Cheerleading
5:00 p.m. Sr. High Boys Soccer, Sr. High Girls Volleyball, Sr. High Golf
5:20 p.m. Sr. High Boys & Girls Water Polo, Sr. High Boys SC, Jr. High Boys XC
5:40 p.m. Jr. High Girls Volleyball, Jr. High Girls XC
6:00 p.m. Jr. High Boys & Girls Soccer, Jr. High Field Hockey, Jr. High Cheerleading
6:30 p.m. Jr. High Football


IMPORTANT INFORMATION REGARDING PHYSICALS BELOW
 

  1. The PIAA physical form, that must be filled out completely, can be found on Governor Mifflin’s website under the ATHLETICS tab.  THIS FORM HAS BEEN UPDATED. OLD FORMS WILL NOT BE ACCEPTED.  Please make sure that all information is COMPLETELY filled out, except for the final page which will be filled out by the physician.  LINK TO PIAA FORM HERE: http://www.governormifflinsd.org/wp-content/uploads/2014/05/Physical-form-for-web.pdf
  2. During physicals, no one will be permitted to enter the gymnasium except the athlete receiving his/her physical.  NO ACCEPTIONS.  We must keep the gym as quiet as possible for the physicians to perform the physicals successfully.
  3. MEDICAL DEFICIENCY FORMS:  If your athlete fails his/her physical, they will be sent home with a medical deficiency form that explains why they failed and what must be done for them to be cleared to participate in their sport.  For example, an athlete may fail their physical due to a heart murmur found by the physician.  The form sent home with the athlete must be signed by the appropriate physician who will clear the athlete after further examination is performed!!  The athlete will not be cleared until this is complete.
  4. If you arrive after the last time scheduled, you will NOT be permitted inside the gym.  NO ACCEPTIONS. DO NOT BE LATE.
