Winter Athletic Physical Information

To the parents/guardians of athletes, grades 7-12, who plan to participate in a winter 2020 scholastic sport,

As we continue to monitor the constant changes throughout our county, we want to be able to provide you with as much information as possible with regard to sports physicals.  At this time, all physical policies will remain the same.  This means that ALL student athletes, grades 7-12, must have a PIAA sports physical performed and dated on or after June 1, 2020.  The ENTIRE PIAA physical form must be completed and returned to the athletic office BEFORE an athlete may begin participating in his/her school sport.  PIAA PHYSICAL FORMS DATED BEFORE JUNE 1, 2020 WILL NOT BE ACCEPTED.

PLEASE NOTE THE PHYSICAL FORM HAS BEEN REVISED AS OF 10/9/20.   IF YOU RECEIVED A COPY PRIOR TO 10/9/20 IT IS OUTDATED

These forms are also available on the Sports Medicine page of the Governor Mifflin website.

Due to on-campus restrictions, Governor Mifflin will not be holding on-campus physicals for winter sports.  Families must contact their primary care providers to schedule a physical appointment, or physicals may also be obtained at any local urgent care facility (no appointment required).  Please make sure that when you go to these appointments you bring along the PIAA physical.

Please be sure to provide a complete medical history on the PIAA form that includes any current medical issues (asthma, allergies and illness).  Additionally, please make sure that the ENTIRE PIAA sports physical packet is complete before returning it to the athletic office.
