7/31: Fall athletic physical make-up date

To the parents/guardians of athletes participating in a FALL 2018 SCHOOL SPORT.

 

IMPORTANT!! PLEASE READ ALL INFORMATION!

 

Governor Mifflin will be holding make-up physicals for athletes (Grades 7-12) who will be participating in a FALL 2018 scholastic sport on Tuesday, July 31 from 4:30 p.m.-5:20 p.m.  Physicals will be held in the high school main gym.  Times for each team to arrive are listed below.  Please try to adhere to these times: 

 

4:30 p.m.     HS Football and Golf, HS B XC

 

4:40 p.m.     HS B Soccer and B Water Polo

 

4:50 p.m.     HS G Soccer, G Tennis, HS G XC, G Water Polo

 

5:00 p.m.     HS G Volleyball, HS Field Hockey, HS Cheer

 

5:10 p.m.     JH Football, JH B Soccer, JH B XC

                             

5:20 p.m.    JH G Soccer, JH Cheer, JH Field Hockey, JH Volleyball, JH G XC

              

 

IMPORTANT INFORMATION REGARDING PHYSICALS BELOW…..PLEASE READ:

  

1.  The PIAA physical form, that must be filled out completely, can be found on Governor Mifflin’s website under the ATHLETICS tab.  THIS FORM HAS BEEN UPDATED. OLD FORMS WILL NOT BE ACCEPTED.  Please make sure that all information is COMPLETELY filled out, except for the final page which will be filled out by the physician.  LINK TO PIAA FORM HERE: https://www.governormifflinsd.org/wp-content/uploads/2018/05/physical-form-2018.pdf

 2.     During physicals, no one will be permitted to enter the gymnasium except for the athlete receiving his/her physical.  NO ACCEPTIONS.  We must keep the gym as quiet as possible for the physicians to perform the physicals successfully

 

3.      Any use of cell phones for photography or videography inside the gym is PROHIBITED.  If an athlete is caught taking a photo or video while inside the gym, he/she will be asked to leave and will not be permitted to complete his/her physical. 

 

4.      NO PHYSICALS PERFORMED BEFORE JUNE 1, 2018 WILL BE ACCEPTED.  AN ATHLETE MUST HAVE A NEW PHYSICAL PERFORMED EVERY YEAR.  

 

5.       MEDICAL DEFICIENCY FORMS:  If your athlete fails his/her physical, they will be sent home with a medical deficiency form that explains why they failed and what must be done for them in order to be cleared to participate in their sport.  For example, an athlete may fail their physical due to a heart murmur found by the physician.  The form sent home with the athlete must be signed by the appropriate physician who will clear the athlete after further examination is performed.  The athlete will not be cleared until this is complete.

 

6.     If you arrive after the last time scheduled, you will NOT be permitted inside the gym.  NO EXCEPTIONS. DO NOT BE LATE.
