GM K-12 REGISTRATION CHECKLIST
PROOF OF RESIDENCY
- Families need to provide two original pieces of proof of residency:
If the student’s parent:
Owns a residence
– Property tax bill, Mortgage Statement, or Recorded Deed in your name
and one of the following
– Country Registrar of Voters; or
Vehicle registration showing current address; or
Most recent utility bill in your name showing current address (cell phone bill not accepted) or
Check stub/statement from wages showing current address
Leases a residence
– Signed lease agreement (identifying the parent(s) and child(ren) as occupants)
and one of the following
– Country Registrar of Voters; or
Vehicle registration showing current address; or
Most recent utility bill in your name showing current address (cell phone bill not accepted) or
Check stub/statement from wages showing current address or
Driver’s license showing current address
Resides with a Governor Mifflin resident (parent not owner of property)
– Residency affidavit – obtained in Online Registration, at each building and Education Center
- Proof of Birth:
Original or copy of birth certificate
If parent names are not on the birth certificate, the following will be accepted:
-physician’s letter stating parent’s names/child’s name and date of birth
-United States passport
-Income Tax form on which you claimed the child
- Health Information
-Complete immunization history or baby book

-Court ordered custody agreement (signed by judge) - School Records
-Last report card and unofficial transcript (Unofficial transcript for middle school or high school students) - Special Education (if applicable)
-Copy of current IEP or ER
-Request of Records form - Foster/Group Placement (if applicable)
-Placement letter from agency
You will be notified by email or phone by your child’s building to schedule a registration appointment.
If you are missing documents at registration appointment, parents have within two weeks after their registration appointment for documents to be provided to the building office. A teacher will not be assigned and/or no classes will be scheduled until documents are received.
