Winter Sports Physicals – Nov. 7

Governor Mifflin will be holding physicals for athletes (Grades 7-12) who will be participating in a WINTER 2017-2018 scholastic sport on Tuesday, November 7 from 5:00-6:20 p.m. Physicals will be held in the high school main gym.  Times for each team to arrive are listed below.  Please try to adhere to these times:

5:00 p.m. – S.H. & J.H. Boys Basketball

5:15 p.m. – S.H. & J.H. Wrestling

5:30 p.m. – S.H. & J.H. Girls Basketball

5:45 p.m. – B/G Swimming

6:00 p.m. – B/G Bowling & Rifle

6:10 p.m. – S.H. & J.H. Cheerleading

 

IMPORTANT INFORMATION REGARDING PHYSICALS BELOW

  • The PIAA physical form, that must be filled out completely, can be found on Governor Mifflin’s website under the ATHLETICS tab.  THIS FORM HAS BEEN UPDATED.
    OLD FORMS WILL NOT BE ACCEPTED.  Please make sure that all information is COMPLETELY filled out, except for the final page which will be filled out by the

  • During physicals, no one will be permitted to enter the gymnasium except the athlete receiving his/her physical.  NO ACCEPTIONS.  We must keep the gym as quiet as
    possible for the physicians to perform the physicals successfully.

  • If your son/daughter played a 2017 fall SCHOOL sport and gave us their physical, you and your athlete just need to fill out the pre-participation physical
    RECERTIFICATION form and hand that in to either the athletic office or the athletic training room.  Please read over this form carefully and follow proper instructions.  Click

  • MEDICAL DEFICIENCY FORMS:  If your athlete fails his/her physical, they will be sent home with a medical deficiency form that explains why they failed and what must
    be done for them to be cleared to participate in their sport.  For example, an athlete may fail their physical due to a heart murmur found by the physician.  The form sent
    home with the athlete must be signed by the appropriate physician who will clear the athlete after further examination is performed!!  The athlete will not be cleared until
    this is complete.

If you arrive after the last time scheduled, you will NOT be permitted inside the gym.  NO EXCEPTIONS. DO NOT BE LATE. 

This is the ONLY winter physical that we will hold for athletes.  If you cannot make this physical and your athlete still needs one completed, I would suggest calling your athlete’s family physician as soon as possible.  Another option is going to Patient First in Wyomissing.  Your athlete can go without an appointment and the physician there will perform a physical for your athlete. Make sure you bring the physical form so the doctor can fill out and sign the final page.
